Go to the ER if you are in extreme pain, dehydrated and dizzy, and running a high fever as those things are potentially life-threatening (plausible pancreatitis, sepsis/intestinal perforation, toxic megacolon) and can be treated by the ER staff and/or get you admitted to the hospital.
If it's frequent bms, blood in stool, and other symptoms that can wait until you can see your gasteroenteroligist then call and see him/her asap (and ER staff will only refer to your gasteroenteroligist for those things, as they cannot treat or manage IBD symptoms).
A flare is a consistent worsening of your UC symptoms (not a one-time bad day) and might include one or more of the following depending on severity: blood on stool, urgent bms, increased bm frequency, pain, diarrhea etc. We can have little flares, moderate flares, and severe flares.
