In a storybook case of UC, a flare dose is 4,800-mg of oral mesalamine a day, and a remission/maintenance dose is 2,400-mgs a day. Often we start at the highest dosage, and then taper down to the lower dosage once we've been in a remission for a series of months.
In practice, some of us might need an oral 3,600-mgs or even 4,800-mgs a day to maintain a remission. We all have different disease severity and individual solutions on what works to sustain us, so it is going to vary a bit among us all. In fact, I needed 4,800-mgs orally, and 4,000-mgs rectally (rowasa mesalamine enema) to sustain a remission for a number of years. I've taken 12,800-mgs daily under doctor supervision (that's 4,800-mgs orally, and 2x 4000-mgs enemas daily) during a flare.
