I feel your pain. With fissures you have to be really disciplined in thinking about
everything mechanically. Like, is this food I'm eating going to cause larger or harder stool? Is it going to cause a lot of friction or irritation on the other end? Am I pushing too hard? Did I lubricate well enough before my BM and topically treat enough after? Am I pushing a lot?
I made goldenseal suppositories with cocoa butter after over a year of trying modern nitro creams. They worked. I'm not a fan of suppositories in general but I was desperate. Goldenseal heals the intestinal epithelium pretty fast. You're still 1 step forward 2 steps back but because they speed healing, the merry go round ended faster for me. You might want to consider herbal suppositories. Herbs actually heal the body, the modern creams don't. Or you can use both.
