I've never used the cortenema, but others have and seem to have success with it.
It's liquid (60 ml) that's inserted in the rectum...the instructions will explain what to do with a picture (at least it's on the ones I use). You will have the feeling to expel it after it's in....squeeze your sphincter and just ride it out...the feeling will pass within the minute or so....although it'll feel like hours.
The doc says for you to use it for 10 days...then what? you should definitely be on the till you are having improvement..then taper them.
But that's a process...sometimes up and down..
The fact that most of your colon has improved is AWESOME!!! A good way to help keep the colon above the rectum calm is to remain on the Lialda. The reason you haven't had much success as you put it...it's because you still have inflammation in the sigmoid/rectum.
What exactly are your symptoms by the way?
If you ignore your oral 5ASA meds, it's possible that it can spread upward again (since your initial diagnosis was throughout).
Doing the both ends approach helps deal with inflammation above and below the sigmoid/upper rectum.
