Hi All,

Well my biopsies from my 1 year follow up colonoscopy came back and I think it's mostly good news. The doctor said that my entire colon looked normal to her (visibly) except a little area near the rectum. The terminal ileum biopsies looked 100% normal (she just wanted to make sure no crohn's). She said the biopsies stated quiescent colitis. I know quiescent means quiet or inactive, but does it mean anything more in regards to the disease? Am I no longer categorized as having pancolitis and now just have proctitis? confused

I think that just because you were diagnosed with pancolitis doesn't mean that any time there is inflammation, it will be in the whole colon. The meds you're taking are keeping most of your colon quiet!

Yes, basically it's not active visually. Healing from top to bottom, good....
The terminal ileum check with pancolitis should be done, not for CD, but for backwash colitis since you do have UC (at least leans to that for now since your doc commented).

If you were to go off your meds...the tendency for the area that was initially affected would also be affected, much more quickly than if your disease were limited, obviously.

So, you have healed from your flare, and with the pattern of UC with rectum inflammation at this point (proctitis)....you need to use the enemas nightly for a while and taper down again. You need to push a bit more with the meds to calm that area....and especially after a c-scope and the clean-out and biopsies, which can exacerbate those areas again.

All is good...the meds are working.

After your biopsy reports come back (make sure you get a copy..!!!), discuss with the doc if you can taper down on the Lialda to maybe 3 daily for 6 months, for example, and if all is good symptom-wise, you could probably go down to 2 daily. As long as you deal with the rectal area well, keep on maintenance and increase with flares to nightly, etc....the above area should still be fine.

The reason I'm suggesting it is that if you were to have issues, you can increase.

The lowest med you can be on for long periods of time is the best, and then increase if you need to.

Maybe your magic maintenance level is 3 Lialda daily....but you should find out rather than stay at the highest dosage.
