It is my understanding that they test for dysplasia, which is often a precursor to cancer.

mumbai, I believe they say something like - consistent with Chronic Ulcerative Colitis but they don't say it's definitely not Crohn's. The only way to find out whether you have Crohn's would be to have a look at the upper portion with an endoscope. I haven't had this so I feel that I can't rue out the possibility of Crohn' even though my inflammation has always seemed consistent with UC. My GI said she's 90% sure I am dealing with UC from seeing the inflammation and the pathology results. She said if I were considering surgery, we would have to go through the full gamut of testing to rule it out completely.

Can biopsy results be wrong? I don't know if it's silly of me asking but my biopsy results were quite odd. They said possibility of UC, possibility of Chrons. Possible indications of this and that. Nothing quite definite. Is there a possibility I don't have UC? Like I've had 2 colonscopys and endoscopys and both times it was like possibility of this and possibility of that. Nothing definite at all. Is that how it is with everyone?

Biopsies can identify indicators of Crohn's like granulomas or they can indicate the ulceration is consistent with UC but they don't know for sure even after they remove and biopsy the entire colon a person can still have their diagnosis change from UC to CD sometimes many years later. As if UC wasn't enough, the fear of CD hanging over our heads SUCKS.

Gina, I think the message is you can't be definitive of whether it is crohns or UC (unless of course you actually end up with crohns). My GI has always said that ATM, I have UC but he can't guarantee it won't become crohns in the future. I haven't had an upper GI endoscopy, I probably should have one in the future, but I so far have no symptoms of crohns aside from occasional anal fissures.

I was told it's an elimiation process..and if all other things are ruled out and blood testing supports, it's usually one or the other.

UC is fairly consistent in its flaring/healing process, but there is one form of rectal/cecum involvement that can be confusing...leaning more to CD sometimes.

CD would have some cellular differences and goes deep into the digestive lining causing different damage, etc.

CD following UC pattern in early stages can easily be confused with UC.

I think one way of suspicion leaning more towards CD than UC is more the healing process..if it's patchy, maybe more CD than UC.

UC doesn't become CD....it's one or the other.

It’s interesting that you mention that Quincy. I have cecum inflammation (or at least I did upon my last colonoscopy) and my GI said (actually my 2 nd GI said it too) that having cecum inflammation actually makes it much more definitive that it is UC (left-sided UC that is) as opposed to CD. He said that most of his patients that presented with proctitis or left sided UC had slight additional cecum inflammation. I remember reading a medical article about this too when searching for cecum inflammation on google last year too.

I should have worded it better to say it's more likely it's UC than CD...but is that form one that eventually becomes pancolitis or continues as is....

I have seen some on here with rectal inflammation that's only treated with rectal meds..not oral. Interestingly, some had never had a complete c-scope. Rectal treatment was the only one given and not oral...wondering why things never improved.

In truth, I think all UCers should be on an oral just to keep the upper area as calm as possible.

And by left-sided, you mean anything above the rectum, because if it's already into the descending colon...I'd suspect it's on it's way to connecting.

 Yep, by left-sided I meant anything in the descending colon and I couldn't agree more on the oral meds. My symptoms have improved since starting on the oral meds, even if it is at a relatively low dose.

There are several studies like the one you mentioned showing a high percentage of left sided patients with a cecal patch. One study actually stated that cecal inflammation could be a partt of "normal colon health." I don't know what they meant by that, and the statment wasn't expounded upon, but it was interesting nonetheless.
