Rectal pressure during the day and pain during BM

Hi fellow UCers

I have been experiencing revtal pain and bleeding while passing BM (bowel motion), then afterwards i feel this rectal pressure which persists most of the day. It is hard to describe and kinda feels like the anus is swollen. I was thinking maybe internal haemorrhoids but ive read that these are usually painless where as i experience during the actual BM. This pain feels like someone putting alot of pressure on too small a passage; only thing is my BMs are fairly small and in no way constipated. At the same time i do not have diarrhoea BUT sometimes the first bit of stool comes out fairly loose.

My guess at this stage is i have anusitis/ lower proctitis. So started using the mesalazine enemas 8 days ago and continued on them however so far i still experience pain (but less) and still bleed fresh blood.
So today i called my GI asking for a suppository, after reading on this forum (thanks folks!) that enemas somestimes bypass the really lower parts of the rectum. So im going to try the pentasa supp and see how it goes.

My question(s) is:

1) Do the symptoms i described sound like proctitis/anusitis?
2) How long should i use the suppository for?
3) should i continue with the enema as well? My GI said i could use one in am and other in pm. Can i use them BOTH at night?(like use enema first then insert the supp??)

Btw i've got 1g enema, 4g enema, and 1g supp

Proctalgia fugax is a benign condition that is characterized by intermittent, recurrent, self-limited rectal pain that is severe. It is one of several functional gastrointestinal disorders and its diagnosis requires exclusion of other causes of rectal or anal pain.

The above normaly happens while you sleep...but would be worth asking your doc about.

I would use the enema at night and pop in the suppository in the am. You did say you saw a little improvement in the 8 days. Sometimes these things just take time.

Hi...welcome to the forum. I think that the enemas would have been better for you than the suppositories at this point and save the supps for tapering or maintenance or adding in the morning.

You can use the enemas effectively to cover the entire rectum by drawing the nozzle out a bit.

Hmm i don't think it's proctalgia fugax because it does not occur in episodes and is not severe... It is more like a dull ache in the but (literally LOL) that is worst right after a BM and usyally improves as the day progresses.
I'll keep using the enemas and add supps during the day..

It sounds like Hemmeroiids to me have you ever taken a mirror and looked down there.  Hemmeroids will cause alot of rectal pressure, and it feels like your anus if throbbing because the viens are swollen, hemmeroids are definately NOT painless they hurt.   BM's will be skinny or maybe nothing at all because the area is swollen making things difficult to pass.  You will see blood streaking the BM or on the toilet paper. 

t can also be proctitis causing an inflamed rectum, both can cause similar symptoms.

Either way rectal foam or suppositories with cortisone help to decrease inflammation. 

Soaking your rear end in a hot bath with epsoms salts many times a day  helps and it keeps the area clean.

Metamucil helps alot if its hemmeroids or proctitis.  It bulks the stool making it easier to pass therefore your not straining, which will  make things throb. Make  sure your drinking lots of water.

Witch hazel on a cotton ball and stick it at the rectum and leave it there for a bit, helps with inflammation and pain.

Puffed Rice i think it might be haemorrhoids as well (GP told me i had a 'pile')...but can't be sure it isn't proctitis either bc as you said they have similar symptoms...do you go toilet more often with proctitis? Coz im not going more often than my usual BUT i did go more frequently for a day, about 12 days ago, which is why i started the enema - and then felt better after i started it; its been slowly improving ever since.
Could be both though...?

     I suffered with UP for several years prior to surgery.  Try the enemas (Cort or Rowasa).  They helped me in the beginning stages of the disease but my condition worsened through the years.  I reached a point where I could not retain them, but you may have better success.  Good luck to you! By the way, it sounds like proctitis.  I know the feeling all too well.

It could very well be both, but seeing that you had a baby last year  it could definately be hemmeroids.  Thats when I realized I had them, from pushiing to lifting the baby, being  on your feet the viens get swollen and they swell and hurt.  Either way soak your butt as often as you can in hot water and epsoms sats and keep up with the enemas.

As for going to the toilet  more often with proctitis sometimes you will go less because the rectum is so inflammed and irritated that stool does not pass.  Thats why it's so important to take metamucil fiibre daily so it bulks the stool and makes it easier to pass.

Hey all...just a quick update that my symptoms (?proctitis) have resolved after using suppositories (1g per day) for 3 days... i continued using them just to make sure it's gone but yeh definitely the supps helped alot more than the enemas.
