Can somebody explain the starting and stopping stuff? Do you basically have to use these drugs forever ?
The fear in stopping/starting these medications is your body can develop antibodies to the remicade, simponi, or humira. So, when you start the medication after a lapse, the treatment is either not as effective as it initially was, or you have no response at all to it. The three medications are different enough on a molecular level, that one can develop antibodies to one medication, but not the other two. So should one have antibodies for Remi, they can take Humira and they'd be fine; no immunity to it. Prometheus Labs has a blood test that looks specifically for antibodies for each med.

But, I do have a question...another GI dr I ran across was kinda of perplexed MY GI went right to Simponi instead of trying Remicade first. Not sure if it was an "old school mentality", or what. Maybe because the profile on Simponi isn't proven long term yet like Remicade or Humira
Different GIs have different approaches. Many start with Remicade first, try Humira second, and then Simponi third. Certainly they are ordered based on approval date. My GI said he takes the clinical study results with a grain of salt. He recommends the medication that he sees as the most effective with his patients. The bulk of his patients are on Remicade. Some are on Humira. Only a few are on Simponi as it's still pretty new and hasn't been approved for all that long. My GI said the results for Simponi look better than Humira so far.
