Inflammation near appendix AND proctitis

It sounds like you have a recognized variant of UC, the cecal patch inflammation with an area of left colon involvement besides. This has been discussed occasionally in past threads on the forum, but I don't think a lot of members have experienced it personally. I don't recall anyone having reported that their doctor said it tends to happen in connection with proctitis, so it may be your doctor's particular experience in medical practice or clinical training to have encountered that type of UC inflammation often enough that he can comment on tendency in patient response to medication. An adult colon varies in size among individuals, and my gastro (who subspecializes in IBD treatment) has commented that some IBDers have more complex twists & turns in their colon than others-- so don't worry that you're some kind of freak case. Maybe the cecal patch inflammation caused some pressure to build in your ascending colon and that led to development of diverticula, which crop up in otherwise healthy people in the left colon due to repeat straining when passing bms. (As with chronic constipation.) Were you having bouts of right-side pain before this latest c-scope? / Old Hat (33 yrs with left-sided UC; presently in remission taking brandname Colazal)

That is very interesting that proctitis could have related appendix-area pain. I've never heard that before. However, my initial flare a few years back started with really bad pain in the lower right quadrant, where my appendix would be if it hadn't been removed many years earlier. The pain was so bad that I took a sick day so I could sit at home with a heating pad on it. The bad pain only lasted for a day or so. A few days after that, I started seeing blood with every BM, which I had never ever seen before, and after about 2 months of always seeing blood I finally went to a doctor and got a colonoscopy.

My GI told me that my lower right side pain before the blood started didn't make sense. He was not an IBD specialist. The colonoscopy only showed rectal inflammation, with no inflammation left side or further up. They checked all the way to the terminal ileum, and there was no inflammation up there either. Of course I was not still having the lower right side pain at that time.

Since then, I do occasionally get achy pain in the appendix area. However, I know there's plenty of scar tissue around the old appendix site (it ruptured before it was removed) so it could be adhesions and unrelated to UC.
