That is very interesting that proctitis could have related appendix-area pain. I've never heard that before. However, my initial flare a few years back started with really bad pain in the lower right quadrant, where my appendix would be if it hadn't been removed many years earlier. The pain was so bad that I took a sick day so I could sit at home with a heating pad on it. The bad pain only lasted for a day or so. A few days after that, I started seeing blood with every BM, which I had never ever seen before, and after about 2 months of always seeing blood I finally went to a doctor and got a colonoscopy.
My GI told me that my lower right side pain before the blood started didn't make sense. He was not an IBD specialist. The colonoscopy only showed rectal inflammation, with no inflammation left side or further up. They checked all the way to the terminal ileum, and there was no inflammation up there either. Of course I was not still having the lower right side pain at that time.
Since then, I do occasionally get achy pain in the appendix area. However, I know there's plenty of scar tissue around the old appendix site (it ruptured before it was removed) so it could be adhesions and unrelated to UC.
