bleeding again..on pred..how?

I actually haven't really bled in over a month or more. Switch to Lialda a few weeks ago from generic colazal. Still doing canasa nightly. On 20 mg of pred..Thought forsure i was heading into remission, feeling pretty good....second BM today blood. No real urgency this morning, Been going 1-2 times a day... Dont know what i changed, nothing really...This is BS. back to the old drawing board i guess.

What's the tapering method of the prednisone?

I suggest you get on rectal retention enemas at this point to see if things improve. You could try a foam steroid enema that might give that extra boost while tapering the pred.

What's the dosage of Lialda...4 daily?

The last part of healing in the rectum is a pain. You could use the Canasa during the day and enemas at night.

HI Quincy

I havent started tapering the prednisone yet. he prescribed me 20mg for two weeks then taper down to 10mg for 2 weeks. Lialda is 2 a day, i take in the morn...So strange was having pretty good BM's a week ago, today they are skinny and bleeding..So frustrating.. This morning woke up to with no urgency, had my coffee everything was fine, then all hell broke loose.

heres what im taking, dont know why my sig isnt showing up

Lialda 2 a day
Pred 20mg
MMJ/Simpson Oil
Canasa at night
Phillips Probiotic
Mens One a day multi vitamin
Vitamin D

What was your dosage of Colazal?
If you haven't tapered the pred yet, you're not ready to...and if you do, I'd drop to 15 rather than 10.

OK thanks..my colazol was 6-9 a day i was taking. But it was the generic and i wasnt feeling very good on it so I switched to Lialda. Maybe i should be taking more of that? not sure what to do, but the enemas sound like that going to be the next step

It won't hurt to up the dosage to 3 to see how that is for a week.
Did the doc start you at 2 and then say to increase if you need?

At the very least as well....increase the Canasa to twice daily. 1000 mg daily isn't enough rectal dosage.

I'm guessing that coffee isn't something new for you but I couldn't help but notice you said you had your morning coffee and then all hell broke loose later on. Many of us would have all hell break loose after drinking coffee. I have seen before where UC'rs have something bother them that did not bother them before. Coffee may now be something off limits for you. Just a thought.

Thanks. Yes ill try 3 Lialda and see what happens. No he didnt say to up it.

Well ive been drinking a cup of coffee and everything has been fine, also i drink tea if im flaring really bad but things have been going really good, i thought. So this is kinda left field really. Seems so day to day lately. Sometimes have to go real early in the morning then other times, its later in the morn 9am-10am and only one BM, thats what happened yesterday, oh i did notice a tiny tinge of blood yesterday on the tissue and thought oh man, but thought canasa would knock it out..it got worse :(

coffee is a natural stimulant to the gut, so personally I don't see a big deal if you expect to have a bm in the morning. Plus, you're obviously flaring.

Flares progress, and using rectals aren't going to out it quickly, so keep that in mind.
If you have a bm against friable tissue in the rectum, you'll bleed. You're not out of the woods quite yet.
Patience is a difficult thing when healing from a flare.

Just as an aside, my brain's been in an emotional state for a while, and I'm now paying for it in slight urgency and thinner stools. Could be IBS, but with some rectal discomfort during a bm, I'll lean towards extremely mild inflammation and start the enemas nightly. I won't see total improvement for about 2 weeks. So keep that in perspective for how we sort of compare. You're in an obvious flare....but if I left this and waited, by next week I would be as well.

Thanks quincy for the suggestions. Ya i knew i wasn't out of the woods, but man i could really see the light !. Then this is just a setback as i see it. Its strange that the pred isnt really knocking it out, because when i started taking it after a few days i could tell it was really helping and heading in the right direction. Even though it is a low dose. anyway, sorry for the rambling, im just confused and pissed. lol

There's someone on here somewhere who purposely uses coffee to empty themselves out in the morning so they can have a productive rest of the day without having to worry about bathroom trips, outsmarting their UC.

Just know that in regards to coffee, yes it's a natural stimulant but that doesn't mean it sends everyone that drinks it (healthy guts or not) off to the toilet, it never affected me that way before IBD or after IBD and it doesn't affect my healthy gut sisters with their coffee either and one sister drinks quite a bit of it daily and she has nice normal once a day poops just like I use to.

2 Lialda a day is a maintenance dose in my opinion. When flaring I take 2 am and 2 pm. 20mg of pred is a bit on the low side too but I agree with Quincy in regards to the rectal meds.

I have heard that it is possible to use two Canasa at once. That might help knock out rectal inflammation.

The full dose of Lialda is 4/day...I am taking that now, 2 in the morning and 2 at night. You might ask about that.

Hi thanks for the replies and help..ya i think ill call my GI tomorrow and see what he says about taking 4 instead of just 2 a day and add canasa during the day. YA i just read on the lialda site 4 a day is for flaring 2 day is remission..

For me the only rectal meds that ever work is cortifoam steroid based rectal meds...have you ever tried them jazzgtrl4? If not you might want to ask your GI about them...they don't go into your blood stream the way oral pred does and they tend to work quickest and best for me...I've been on them as long as a year before tapering off of them and no side effects (I let my proctitis go too far and that's why I had to be on it so long that one time)...good luck, I hope you find relief soon!

I think you should see about increasing the Lialda. Maybe i have the timeline mixed up but if you haven't bled in over a month and you've only been on prednisone for 2 weeks, why did you start prednisone? What were your symptoms?

Notsosicklygirl: Ya im going to increase my Lialda. I'm not sure really why he wanted me to start it, maybe to push me over the edge of simmering, into remission. Ive only been on pred about a week and half..I actually waited about a week after he prescribed the predisone to start it, wanted to see how the lialda would work. I figured it was lower dose so it should help and i started it.. I was still having symptoms though, little mucus, some urgency. Stools were skinny sometimes, sometimes looked great...a little blood if i remember correctly but it was just a slight tinge on the tissue, not like bright red, dripping in the pot when im flaring bad, that hadn't happened in months...Id use a canasa and then it was gone... I dont know, Ive been stressed out with a bunch of stuff as well, selling a house,work, all kinds of things i guess..

oh i was on pred back in oct for a nasty lung and ear infection 60mg. My GI knew that, maybe thats why he wasn't hitting me hard with a higher dose..don't know. hes pretty vague and in and out the door, time for a new one soon i think.Ive never taken predisone for UC>
