Remember, you can always use the steroid enemas in conjunction. Foams are easier to retain during the day and use the mesalamine enema at night.

It all takes time to deal with a flare, and it's a learning process. Hope the mesalamine enemas work for you.

What's the dosage of the Asacol? And...what's the extent of your UC...limited or throughout?

Thanks Quincy. At the moment I'm only on 2 asacol in the morning and 2 at night. I used to take asacol suppositories at night but didn't work and kind of made me worse. But that was last yr when I was really bad and was diagnosed. The enemas I think work better but will see how it goes. This is all new and I need yo find what works for me. I really hope I can. I hate living like this one day good and the next bad. At the moment I'm away and flaring so gone back on the entocort enema for 5 days until next wed when I have my scope. When I go on wed will mention the rowasa and the cortifoam. Is that a maintenance you can be on for good?

Is the asacol 400 mg or 800 mg pills?

The asacol suppos probably didn't work and made you worse because it wasn't enough medication. Worth a try first to do the mesalamine enemas and see what they can do for you. If they aren't for you, then the steroid enemas are what can work for you.

The steroid enemas aren't a maintenance medication...treatment, yes. The mesalamine are treatment and maintenance...but you have to go through a tapering process to maintenance (which I've found staying at every 3rd night is good...but I used to taper to once a week). I've been at this for over 26 years and not used anything but mesalamine (except a steroid foam once the first year given to me by another GI).

I'm on 800mg 2 2x daily. I see you are on sofalk enemas. Are those the same as Rowasa? I'm really hoping it will work. I have mild/moderate uc. Started off 5 yrs ago that 10 cm of my lower left colon was inflamed. Then last yr was a bad flare. I can't reefer what the dr told me, just said it was mild / moderate. I think because I'm on such a strict diet my flares now are so mild. Bleeding, mucus and some constipation. I don't really get the cramps and fever like last yr. I know it's lower down because the bleeding is bright red. Been taking this entocort enema for 4 nights and flare has settled. So hoping the other enemas will work. If the inflammation is in the rectum I'm sure the enemas would be better to control it than up my dose of the asacol tablets?

Yes, the Salofalk is the same as Rowasa. Available in Canada, UK and I think maybe Australia.
It's also the same as Pentasa, but Pentasa enemas have 100ml liquid compared to 60ml of Rowasa and Salofalk.
Rowasa and Salofalk have a 2g and 4g dosage.

You're on a moderate dosage of oral, and no...it won't control a rectal flare. Diet won't determine your flare either...but it can help with symptoms that would be food related (difficult to separate the two when flaring).

The constipation can help determine where the limitation of your flare is...which is rectal. Since you're bleeding, the tissue is in a more advanced state of inflammation.

You could also consider to use the enemas and supplement with suppositories during the day....there are a few options.

Fibre supplements can help bulk and soften the stool helping for somewhat easier passage...but food changes can help as well such as adding more foods that will loosen/soften. I tended to take higher vitamin C which helped quickly...better looser than harder stool, ugh.

It's good you're seeing improvement with the enemas.

Rowasa, no....but I suspect you have Salofalk or most likely Pentasa enemas.

Remember they both come in different dosage strengths.

I'll see if I can find any info.

Mesalazine is the active ingredient of Asacol Tablets (Nappi Code: 783668-007),

Asacol Enema (Nappi Code: 824127-005),

Asacol Supps (Nappi Code: 824135-008),

Mesasal Tablets (Nappi Code: 707496-001) and

Pentasa Tablets (Nappi Code: 890775-004).

I found these mesalazine (same as mesalamine) products available in South Africa...

So, Asacol enemas seem they're available (but it might be a foam or liquid... but if Pentasa tablets are available, maybe it will be in enema form as well.

The best thing you could do is to call your pharmacy (talk to the pharmacist) and see what they can get for you...then when you talk with your doctor, he can write you a 'script for that.
