Quiescent IBD?

Does anybody know what quiescent IBD means? I saw my multiple biopsy results last week and despite very mild inflammation in sigmoid, the report had in capital letters "Quiescent IBD" which my GI highlighted to me and told me to move on from this flare.

What does that mean?

quiescent means quiet and not problematic, but it could mean responding to treatment.

Mild inflammation does not mean quiescent, although some UCers can have mild inflammation with no symptoms. It's subjective...but your doctor might believe that a flare starts when you are bleeding. I really, really, really disagree with that thinking because it's careless treatment.

Move on, yes...but be proactive and treat what mild inflammation you nave since if you don't, it will move beyond the state it's in...and that's not quiescent.

I wasn't having enemas and stuff at the time of the biopsy. I think you're right I should treat it, the GI's issue is my symptoms are that of piles and he doesn't want over treat something as the biopsies were not significant.

the mild inflammation was in your rectum and sigmoid or just sigmoid...not the hemmies, correct? Two issues doesn't mean choose one ignore one. They both cause you symptoms.

What bothers you the most...the fact that he said you might become dependent on the rectal meds? or that he's not believing you need to treat the mild inflammation? or that you're confused about what to treat?...?

Inflammation in sigmoid, the size of a 10p piece very mild which he said has probably cleared up as symptoms have gone. Some mild proctitis near piles, but that could well be irritation.

He is treating the rectal inflammation, 500m suppositories day and night, and I am on 4g Pentasa by mouth - which completely got rid of all symptoms. He says the minor bleeding with no other symptoms is the piles unless diarrhoea, mucus, urgency and blood all arrive at once. I normally don't get D so the latter three need to happen.

The issue I have is blood freaks me out and I feel I need to treat it instantly, so yes, I will do the enema tonight at least for a couple of weeks. He says my inflammation is mild, and bleeding is always from piles - so ignore mild bleeding. Thats the dilemma - and he is a VERY cautious GI normally.

Are you sure you don't have CD, because you're not fitting a pattern of UC, actually.

Not that it matters..mesalamine/mesalazine is used for both.

The rectal inflammation could be a flare reaction from the hemmies if they're inflamed. Yes. I have had that happen from external hemmies that were pretty bad for a while.
Intermittent bleeding would probably be from the hemmies when on the toilet paper..or from the patch in the sigmoid if there's an imbedded amount in stool (since it sits in there).

As I suggested in the other post, maybe switch to Pentasa enemas and see if that helps. It might sit better in the sigmoid to deal with the patch in there, and might not give you lung symptoms.

Once you figure out your symptoms and separate them...maybe that will help. Do you have proctocedyl products in the UK? Those supps and ointment are amazing for hemmies and they would help to deal with the mild rectal inflammation as well.

Yes I have CD. Totally - mild CD. I am taking Hemmie suppositories - and will continue on them tomorrow. And you're right the symptoms do sound more like Hemmies and the rectal inflammation is from Hemmies too - it's right in that area - maybe a 2 week burst targeting the small patch of inflammation is reasonable - at least then I'd have had a go at it.

My symptoms right now are, noisy gut, and very mile bleeding on tissue - that's happens maybe once a day - I have 2 or 3 BM's a day.

I guess after my long-winded encouragement the point would be as you said..target the areas for a length of time and see how it goes.

Antispasmodics are a good thing too...I use Bentylol (Dicyclomine), started with 10mg and graduated to 20mg.

I have 1-3 bms daily..depending on what I eat and how much the day before. When I'm flaring, urgency and butt spasming is huge with each bm and the spasming between bms. Add lots of gas, cramping and stomach pain as well.

Hang tough...keep us posted how this weekend goes.
