Fissures and UC are 2 totally different things, although often people who suffer with UC end up with a fissure. I have had fissures and I have UC, and while painful, fissures never really caused me too many other symptoms. They cause blood on the toilet paper, sometimes in the bowl and the type of pain that could make ANY creature cower in fear, but they do not cause you to have an accident. They don't cause internal pain, like cramping or pressure in the abdomen. The pain from the fissue is all localized right there around the anus and it's like squeezing lemon in the most painful, deep, sensitive cut each time you pass anything. They are really horrid. Hot sitz baths can provide some relief, but really, he should see someone and get properly diagnosed to find out whether this necessitates something stronger than OTC protectants.
As for the other symptoms that he's been experiencing, it could be m any things. I think an appointment with a GI would be best to get to the bottom of it. It could be a temporary GI bug, food poisoning, it could be a bacterial cause - stool tests would be helpful, it could be his UC flaring up. There's no way to know without tests. Maybe he will get lucky and it was some type of food poisoning that will clear on its own.
Oh sorry, I just realized you mentioned he is in the middle of a year long flare up... That means he needs to go to the doctor and find out what's going on & add additional medication, get stool tests, blood work up.
