Flare on Simponi?

So I've been on Simponi with an azathioprine 50mg/day chaser for a year. It's been like a miracle. Until 3 days ago. All of acsudden the blood is back. I can't pinpoint a cause, just stupid UC doing its stupid UC thing. So dejected right now. I just really wanted this to work. I'm wondering if anyone else out there has started to flare while on Simponi. What did you do to get it under control? Did you have to switch medications? I really don't want to go on Entyvio and am allergic to Remicade. Seems like the only options left are Humira or surgery.

I think BaconGirl flared on it or it wasn't 100%, she used some alternative treatments like nicotine patches.

I think that you should call your GI. Maybe you could increase your Aza dosage to 75 mgs.

There is also Cimzia. I'm surprised that Simponi works for you if you're allergic to Remicade.

I think I would try to get stable without changing meds. Maybe an increase in current meds, or rectal meds. Humira might not be a bad option, but changing is always difficult. Can you request stool tests? Entyvio is a good option too, not sure why you don't want to go on that specifically? I had surgery and it's been so wonderful not worrying about what's next.

Is it just blood or are there other symptoms like urgency, increased frequency, mucus, and less formed bms? I ask as blood can occur from hemorrhoids, fissures, and other things.

I'd call your gastroenterologist and tell him/her about your change in symptoms. I'd ask for a stool test, and perhaps a flexible-sigmoidoscopy is in your future to see exactly what is going on. If not a scope then at minimum get a test to check your body's inflammation levels: CRP blood test or FCP stool test.

I'm not entirely family with Simponi dosing, can it be increased? I'd ask your doctor. I know humira and remicade doses are often increased to get a patient back into optimum therapeutic (healing) range.

Not aware of any antibody to simponi tests. I'd ask your gastroenterologist if there is a test like that.

I'm not sure if you've failed simponi, I'd ask lots of questions.
