I've had loads of fevers during flare-ups. When I started Remicade, I started having spiking fevers about a month after the first infusion - the highest one reached 105C. The next day I got myself to A&E and was admitted. Briefly put on IV steroids, then taken off them by my GI 'cos he thought I had gastroenteritis *rolls eyes*. Long story short, I was discharged and back in again a few days later, in an even worse state and unbearable pain. That time I was put on IV steroids and kept on them. Was in for about 9 days. They did stool tests every day, but no infection was found. An abdominal x-ray showed signs of toxic megacolon.
There's two main reasons why steroids don't work. 1) you are steroid resistant and 2) you have an infection. If it's the first one, you may be a slow responder rather than a non responder. In my case I responded to IV steroids literally overnight, but for somebody who barely responds to steroids it may take up to 3 weeks to see a response. If it's the second one, then steroids will just be masking the underlying infection; you need to be on antibiotics instead.
Are you on opioid painkillers? Opiates/opioids are notorious for bunging people up.
I hope your hospital have run repeated stool cultures. I will lose all faith in them if they haven't done at least one stool culture.
PS: You don't have to stay in hospital for 3 weeks to see if the IV steroids are going to work; you can transfer over to oral pred. The hospital will discharge you once they feel you have been sufficiently stabilised.
