Balsalazide for proctitis

Has anyone used balsalazide for ulcerative proctitis? Of so did it help? I go for my second opinion tomorrow morning. I already tried lialda once a day and apriso once a day maybe if i try one that is three times a day would work better.

Balsalazide, apriso, and lialda aren't going to treat the rectum well for proctitis patients. Rectals (suppository, enema or foam) work better. Pills will help reduce the odds of proctitis spreading further up into the sigmoid colon and beyond.

Is she talking mesalamine or steroid?

Your doctor is wrong. Tell him/her to take their degree and return it to the university that issued it :-P

You're creating multiple posts with the same or similar questions... We're consistent in our answers. Get a second opinion from a gasteroenterologist who specializes in IBD smile

The confusion about long-term use may be related to steroid enemas vs mesalamine enemas, or it may be more related to the fact that it is known patient non-compliance is very high with long term use of rectal meds. t takes self discipline to stick meds up your but month after month and year after year. it is also expensive.

In some HMO and national care contexts any long term use of expensive meds is discouraged. I would understand if an HMO told you that you could not have enemas long term.

But have a conversation with you doc to straighten out these issues, or do the 2nd opinion. It is up to you to clearly state the questions about why long term mesalaming rectal meds is not recommended and to ask your doc to speak to patient-groups who say otherwise. FInd out why they say what they say.

At first i wasnt interested in doing the mesamalmine enemas but i told her yesterday that i am willing to give it a try but she says they arent for long term use. I am ok with doing them for as long as i need to if it helps my proctitis. Im sick of feeling the way i am feeling right now.

if you want us to help you, you really need to answer some questions -

what kind of doctor are have you been seeing and what kind of doctor will you see next ?

how long have you had proctitis/UC ?

are you in the US ? is so, where ?

please create a signature - like this:

11/08: ischemic colitis and scope perf colon. 12cm colon/ileocecal resected. IV antib:sepsis.
01/10: Dx: Mod. UC pancolitis. Rx: Lialda 3x.
02/11: Major flare w/antib:sinus. Rx: 40mg Pred taper. 6mp.

Im a 36 year old female. I live in the US. I currently see a gastroenterologist. She has only been a dr for about 2 years (not sure if that makes a difference) I will be seeing a different gastroenterologist from another practice tomorrow. From her credentials she has been practicing for over 20 years. I was diagnosed with ulcerative proctitis jan 2015. I was on canasa twice and seemed to go away. Then symptoms came back in july had a sigmoidoscopy and got diagnosed with uc. Was on lialda for 2 months had a flare got put on uceris for 2 months. During that time had another sigmoidoscopy and she said i was in remission but still had pain and diarrhea so she diagnosed me ibs. Am on 25mg of amitriptyline. Went on apriso for 1 month and am currently having a flare with my proctitis. Rectal pressure and urgency.

Ok. Besides uceris foam is there any other steroid enemas or suppositories? Uceris seems to make me worse butbthis canasa the dr has me on doesnt seem to be helping alot. Im still having rectal pressute throught the day.

There are generic steroid suppositories, steroid rectal foams, and steroid enemas available.

When we're flaring most foods bother us. Try simple, bland foods without a lot of spices. Chicken, rice, potato, soups and broths and such. Well cooked food is much easier on the digestive system than raw. Caffeine and alcohol might make your symptoms worse.

So i saw the second opinion today and long story short she said before she can treat me she has to do her own colonoscopy. She did however agree with my other dr that toy cant stay on the enemas and suppositories long term. She said you can stay on them for a while but once things calm down to stop taking them and go on a pill form. Im actually more confused now which dr i should stick with. I did talk to my original gi because i told her that i am hesitant to go on the immunomodulators especially since she me tion my uc is mild. She said she is open to me trying one more mesalamine pill.

Now what the hell do i do now? I left the 2nd opinion in tears. I was hoping for a treatment. But all they want to do is mote tests.

Did the doctors say why they don't think you can be on rectal meds long term? Doesn't make sense to me. You should be on both oral and rectal meds at the same time. Once you are in remission you could cut back on the rectal meds, but still use them occasionally for maintenance. Stay on the oral meds daily even in remission. When you were on Lialda how much were you taking? Have you ever had a colonoscopy?

What city are you located in? Maybe you could start a new post and ask for a GI recommendation. You need a gastro doc that has extensive experience with IBD -- not all gastros are alike

You have to be persistent and your own advocate with this disease. It can be frustrating until you can find a doctor you can work well with.

She said i can go on them for a while until things calm down then i could get off them and stay on a pill indefinitely. I was on two pills of the lialda. I had a colonoscopy dec 2015 but had two sigmoidoscopies in aug 2016 and oct 2016. Apparently i have proctosigmoidisitis.

I have been using canasa daily for over 10 years along with oral colazal. You most definitely can use rectals long term. It is not unusual for a new GI to want to do his/her own colonoscopy. When I switched dr's the same thing happened.

I fugured that but i just had 3 scopes in ine year i really cant afford to do another one. My insurance isnt great. I see you are on uceris. How long have you been on that for?

I should take the Uceris off and put in the past meds. I was on it last year for 3 months. Everything else on my signature is current, I just never removed the Uceris from the current meds. It worked good at the time. Got rid of the flare. The flare was in my decending colon/sigmoid area and was moderate inflammation. I caught it in the early stages with Uceris. If I was in a severe flare it would not have worked...I would have needed to take prednisone. The only meds that keep my rectum in remission are the rectal meds. I take the oral for the rest of the colon since I have pancolitis.

Oh ok. See my dr said that the pills should take cate of the rectum. But i would think you would need the rectal meds too.

the most delayed release can when there is not much D.

some people do taper off rectals. i did because i could. i was not told i had to.

some stay on a lwer dose for years. could you be confusing what the docs are saying can do with must do?

drop the 2nd doc. if you have a recent scope they can watch the video and read the path report. there is not a need for a new scope unless there has been a large change, or a couple of years
