Best hemorrhoid/fissure treatment(s)?

I'm all but convinced that I'm experiencing a BAD case of either hemorrhoids or fissure at this point.

I've started using lidocaine 5% ointment + Preparation H wipes, and they have turned my extreme sharp pain into intense itching, which leads me to believe it is, in fact, some sort of hem/fissure scenario.

That said: I had trouble sleeping last night due to the discomfort/itching/stinging. Tylenol and those treatments help to diminish the sensations, but is there anything else anyone can recommend? Do sitz baths work well?

Any input would be appreciated. Thanks in advance.

Unfortunately I have had a few bad fissures due to constipation over the years (and external hemmies) so FWIW:

Baths are a huge help! Every chance you get - and Maximum Strength Desitin or Boudreaux' Butt paste - will help quickly with your fissure (put on overnight). Stay away from anything with witch hazel if it's a fissure, e.g., Prep H wipes - witch hazel is lovely for hemorrhoids but very bad for fissures. Alcohol is bad for either.

If it's hemorrhoids only, Tuck's pads, refrigerated (not kidding) will be of help to you.

In either case, look for the Cottonelle wipes that are alcohol free. Blot versus wiping. Be careful of harsh soaps - let the shower spray rinse that area well for clean without irritation.

Finally, you may find some help from the Desitin for external hemorrhoids as well - Proctofoam also helped mine. Good luck!

How long does the pain/soreness usually last, Fletch10?

It's now been about 1.5 weeks since the symptoms started and I'm on day 2 of extreme discomfort. I drank a glass of wine 3 nights ago, maybe that exacerbated things.

A cube of ice applied to a thrombosed external hemmie helped me in addition to the other things mentioned. For a fissure, the best thing I have found is boiling flax seed and applying the musciligen. I was in pretty bad pain for 3 days or so before the hemmie decided to deflate. Not sure what actually did the deed.

unlikely to be wine.

I have only found 1% lidocane or dubicane. What brand is 5%?

A fissure really stings and can feel like a knife when passing stool and wiping. A hemorrhoid, especially internal is more associated with a duller pain and swelling. Of course external you can just plain feel or see.

Psyllium seed powder (1/2 spoon in the AM with warm water) made my hemmies disappear.

I just bought the OTC lidocaine at Rite Aid, the tube says 5% and "the strongest OTC lidocaine," so there it is.

I am starting to think its a fissure based on the descriptions. I couldn't imagine myself boiling flax seeds and then properly applying it to my rectum. Are there any OTC or medical remedies that can be used to treat fissures?

How long does pain from fissures usually last?

The pain from a fissure is no joke. It's excruciating when going to the bathroom - you KNOW when you have one. If you stick with frequent baths, apply the extra strength Desitin at night (or similar) and aren't straining/pushing to defecate (tough with UC if you have tenesmus!), it should self-resolve in a few days. If you have to push that can exacerbate things and prolong/make it a chronic problem. Deep breaths while pooping are your friend!

I took a sitz bath and also showered and made the water a bit hot and let it stream over my anus and both provided signicant relief

I think it's safe to call this a fissure by now

ALTHOUGH I HAVE WITNESSED WHAT A COLOSTOMY INVOLVES AND MAINTENANCE REQUIRED I AM A MAN WHO HAS LEARNED HOW TO ADAPT DUE TO BROKEN NECK,BACK AND RIBS AND MY SPINAL CORD INJURY MY DAUGHTER SAID AFTER READING THE KORAN IT SAID ITS NOT THE STRONG THAT SURVIVE --BUT INSTEAD THE ADAPTABLE-- SO TRUE! I DO NOT WANT TO BE FACED WITH THE COLOSTOMY ORDEAL AND IF IT'S WORSE THAN MY PRESENT SITUATION SINCE I AM DOING PRETTY WELL WITH USE OF MIRALAX AND PROBIOTIC--PS I HAVE USED DELZICOL AND ASACOL AS WELL AS STEROIDS BUT DISCONTINUED BECAUSE OF COST OF CO-PAY AND THE FACT THEY ALWAYS MADE FECES RED COLORED WHICH I BELIEVED TO BE BECAUSE THE MED ITSELF IS RED WHAT A DILEMMA // LOL// DUANE IN AZ
