DIY FMT - How long do you have to lie down post administration?

I am seriously considering DIY FMT. I would like to know from those who have tried, how long do you have to lie down before you resume daily routine post administration of fecal enema/FMT?

When I administer the enema, I lie down at least one hour but even after that, I manage to excrete some of the enema.

I have FMT at the hospital and the recommended time is 20 mins

Did you get an urge to defecate after you got up? I usually lie down for at least an hour after enemas, planning about the same for FMT too. And almost always after I get up, I have to make a trip to the restroom to out whatever was left inside..

The specialist at the Hospital that I go to for my FMT say that after 20 mins the FMT will have done its job so even after that if you lose some thats fine,

In terms of how long to hold it, I've heard as short as 20 mins or as long as 4 hours. I don't think we know the magic number yet. I do mine at night now so I can keep it in for 8+ hours - not necessary to do it this long but it works well with my routine. In the morning it's a solid formed stool. Not sure how many hours it has to sit in the colon before it changes from a slurry to a formed stool... maybe 2 or 3 hours? At that point it's much easier to hold in if you need to get up and move around.

The longer the better.

Sometimes I was able to do it overnight, sometimes only 20 mins. The bacteria transfer very quickly.

The main thing that works against it is inflammation at the site of the transplant because they won't populate inflamed tissue that is having an immune response.

This is exactly what I was thinking about.

When I use hydrocortisone enema, the blood disappears for a couple of hours but then its back so the inflammation remains. Don't know if I should just start oral steroids but I am dreading the effects of Prednisone.

You need to downgrade your inflammation a lot for FMT to have a higher chance of success. It's not that you can't do it while in a flare, but the success rate will be lower. Also there is a chance that the introduced bacteria will aggravate the inflamed tissue and make it worse. That's what happened to me. I developed an abscess that needed surgery, and then it turned into a fistula. My case was weird but my point is, you have to be careful.

FMT seems to have the best effect on people who are close to remission... like 80% better and they need that extra 20%. But there are no hard and fast rules. It might work for you. I'm just telling you my experience + what the FMT community online has said over the years.

A lot of people use prednisone + FMT. The problem is that prednisone stops the inflammation but the intestinal lining isn't healed yet. The gut flora live on the mucosa, which is the healthy layer of mucus that develops on top of a healthy, in-tact bowel lining. During inflammation that lining is not in tact and the mucosa is broken in places... so there's nothing really for the bacteria to live on. If you have ulcers, then the FMT bacteria will be delivered on top of the ulcers and just irritate them.

Makes a perfect sense. I may try Budesonide, I never managed to lose the weight that I gained 6 years ago due to Prednisone.
