I was on Lialda up until 2015 when my creatinine levels were trending upwards. Ultrasound showed possible interstitial nephritis. They did another challenge dose in 2017 and my creatinine shot up from 1.1 to 1.6. Wondering if anyone else has had this problem. My question is if it's safe to try something like Rowasa in which there will be less systemic absorption. The nephrologist said it's worth a try if nothing else is working but obviously keep an eye on kidneys. But, several GI docs I've spoken to said they wouldn't try it. Probably because they want to jump straight to something like biologics.

Sorry to hear about the reaction. Lialda and Rowasa both have about the same 10-20% systematic absorption, no difference among them for it. I would not expect Rowasa to be any different or better in that regard.

I would switch to a not 5-ASA/Mesalamine option, like a thiopurine(Imuran/azathiopurine/6-mercaptopurine), or a biologic like remicade/humira.
