G'day everyone. My first time here and I wanted to share my story so that it hopefully may help someone in some capacity as well as to receive some support in this hard time.
Here's my deal.
I'm 30 and have been dealing with GERD for well over 5 years in a capacity that was fairly disruptive to my life. I have also been dealing with Sleep Apnea since 2002 (and possibly before). I'm slightly overweight (5'11" and 200 lbs) so all of this with my age and weight and everything is not your typical GERD/Apnea patient, but nevertheless.. i've been constantly fatigued and stressed for the greater part of my 20's. Something HAD to change.
After years of sleep studies, CPAP trials, Anti-Depressents, Anti-Anxiety Meds, holistic approaches, supplements, Excercise, etc etc etc My symptoms have not gotten any better, in fact, they've been getting progressively worse.
For Sleep Apnea i'm currently using a dental device that is working out quite well (please PM me if you would like more info). I've had sleep studies with it in and my apnea is reduced to a very minimal level, better than CPAP, especially because i'm 100% compliant.
However, I still feel fatigued and the reflux was getting BAAAAAAAAAAAAAAAAAAAAAD. THe bloating, the belching, the food coming up, shortness of breath, chest pain, you name it i had it. Except for Heartburn. I've been on Nexium 40mg/day in the morning 1hr before eating for the past 3 years and I havent had heartburn except for when I miss the med for more than 1 day. I had my first endoscopy 3 years ago that revealed a Hiatal Hernia and a stricture near the top of my espophogus (which they popped w/ a balloon). Before that endoscopy i was having a hard time swalling thick foods like Sandwhiches, Peanut Butter, etc. After the Endo it was night and day...
Fast Forward to 6 months ago. My GERD was getting unbearable and I revisited my GI doc and he suggested a series of tests to see where I was at. Tests included:
1. Endoscopy - Revealed Hiatal Hernia again and small stricture (they didnt do anything about that this time)
2. Abdominal CT Scan - Normal
3. Motility Test - Slightly under normal at my LES
4. 24Hour Wireless Bravo PH test - This i took after stopping nexium for 7 days and it was HELL. CONSTANT Heartburn and reflux. Report showed Severe Reflux..duh.
5. Plenty of Bloodwork - all set there i guess too
So, they basically told me that the Nissen procedure was my only option (or live with what i've been living with, so basically that wasnt an option because I was miserable). So, after much research on these forums and others, med sites, friends/family in the medical field, i decided to go ahead with it.
I had my Nissen Fundoplication with Hiatal Hernia Repair sugery done on Wednesday March 11th (4 days ago at time of writing this) and am now in the recovery phase. Here's what's been going on since.
- Day of surgery (Day 1) - My procedure was done Laproscopically and was done in just over 2 hours. Doctor said everything went great actually he found a little Diverticulum (pocket?) towards the bottom of my esophagus. He said he just wrapped around that and everything else was pretty textbook. He said that if that went untreated that could have caused some major issues if that had teared. It was nice to receive some confirmation that this was a good choice. He also said he uses a presized tube shaped thingie that he wraps around to make sure the opening is uniform and doesnt get too tite or loose. HE said he's done between 130 to 150 of these procedures. He's a General Surgeon / Vasular Surgeon that's been in the field for..well, he looks like he's in his 60's so i can imagine quite a while. He came highly regarded by the hospital as well as my girlfriend who works directly with him as his OR Nurse.
Anyways. After I woke up from surgery i was in some pretty bad pain, which my Morphine PAC was able to deal with pretty well. I pretty much just slept and took very very small sips of water. I had to get up every hour to take urinate and it took between 5-15 minutes to get out about 300ml. I had to sit down and pee into a girl pee catcher..heh. They said this is a side effect of having to have a catheter in during surgery. My throat was sore also from the intubation. The first night in the hospital i got NO sleep at all between all the pee trips, forced walks around the floor and hourly vital checks and requests for water, gasX, etc. Oh yeah, they had to ORDER GasX and it took 4 hours.. nice.
- Day 2 - (Day After Surgery) woke up after about a whole 1 hour of sleep (if that) and had to take another walk and get some more vitals. I was still having a hard time peeing so they gave me a straight cathetar and drained 800ml of pee out of me. WHOA! I was only getting 50ml of saline per hour (for 8 hours = 400ml) and drank 1/2 c of water ALL NIGHT. It dont add up! I also peed 4 more times at 300ml each by the afternoon.. what the!? My pain really was focused around my stomach (incisions) and back (from the bed) and morphine was helping quite well with that. I didnt want to overuse it so i pressed the happy button only 4-5 times. I had about 5 teaspoons of vanilla ice cream, another 1/2 C of water and about 1/4 C of apple juice all day until about 8pm when I was released (doc did his rounds and released my home with my nurse girlfriend. Probably wouldnt have happened otherwise since i wasnt eating anything). I was finally able to pee OK though and i actually farted a couple times and burped a few times and wasnt in any significant pain so i was comfortable with going home. No way i wanted to sleep in that bed another night! I got home and took a hot bath, took some gasx, 1/2 C of water and went to bed - slept like a baby, probably because i was so exhausted from not sleeping the night before
- Day 3 woke up feeling OK, still pretty sore around the midsection and very carefully and slowly moving around, getting up, sitting down, etc. Spent the day pretty much laying on the couch watching movies and playing video games. Couldnt really focus on anything other than the discomfort of swallowing. Here's the worst thing that's been happening to me since surgery. I CANT SWALLOW! even SMALL sips of water feel like they're getting STUCK. WaTER! i took my nexium with water and it took everything i had to relax and allow it to go down. Painful and very uncomfortable but it went down eventually. Small sips of water throughout the day. I even braved it and had some chicken broth for lunch. Very tastey! about 1/2 C of that. I was able to drink 1/2 C of Apple juice too, but still very painful. The doc sent me home with Roxicet (liquid percuset) but I have yet to be in pain enough to take that. It's VERY good medication for pain and i would HIGHLY recommend you request this from your doc if you are in pain after surgery because anything in pill form is NOT COOL! I had my girlfriend buy me some liquid Motrin (kids) and tried to take 2 T of that. I ended up regurtitating one of them, kinda scary but i guess it's to be expected. I am also BURPING after EVERY attempt at ingesting liquids. In fact, i cant lay back down until i burp. I have to kinda pat my chest in order to work it out. It doesnt HURT per se but it's very uncomfortable until it happens. Took another hot shower and was able to go to bed no problem. I actually drank 1/2 of a Boost before giving up on it but i need the nutrients and protein.
Before i went to bed i weight myself. I started off at 200lbs, I'm not down to 188! Eesh! That's 3lbs / day! All water weight i'm sure and I certainly had it to lose so like most people this is a welcome side effect..pretty much the only one. Oh, and still no poop...weird..
- Day 4 - Woke up again this AM and felt a BIT better. I decided to test my eating progress and i'm able to get down water a SLIGHT bit better today, which is encouraging. I'm trying to stay patient and positive. Still burping after every sip. No straws i know! I'm gunna try jello today and finish that boost drink. Oh yeah! I pooped finally! I actually high fived myself... i know..
So, that's it for now. I know, long story but i figured if i got pretty detailed people could relate a little better and it kinda serves as a sorta blog for me to document my progress and keep my mind positive.
So, moral of the story.
1. Do your research before surgery
2. Find a GOOD Doctor that's done AT LEAST 100 of these
3. After surgery, plan to lose 10% of your weight and only eat LIQUIDS for 1-2 weeks
4. Ask for ROXICET! (it's liquid gold)
5. Stash some GASX in your bag for when you're in the hospital cuz the dumb nurses wont get it to you for hours
6. be patient and positive
7. visit these forums and focus on the POSITIVE energy, there's a lot of negative crap out there to focus on, but it's good to see people who come back when things are going well and not just the people who are having problems. If you find yourself doing well, please check in every so often to spread some good news around, there can never be enough.
Ok, so i'll keep ya posted. off to eat a delectable lunch, chicken broth! (I ate a HUGE steak and Potatoe dinner the night before surgery as my LAST meal..heh)
