My understanding is that reflux has a number of causes, but the surgery improves symptoms only if the cause is a hiatal hernia or a weak LES (or, perhaps, a strong LES that has too many or too lengthy transient relaxations). Thus, if the cause of reflux is an ulcer, dysmotility, insufficient stomach acid, h. pylori, or candida, surgery wouldn't help.
