Sounds like you are doing what the rest of us are doing. I sleep with the bed elevated, try to stay away from caffeine, pop, coffee, and tea, with nothing to eat after seven. Nexium didn't help me either. I think the doctor thought it was all in my head.
I am taking Pepcid twice a day right now and it is helping as much as the Nexium did. I have had two endoscopies. Both times, the doctor said my stomach is inflamed and that I have gastritis. I can't eat a real big meal. Bulky foods make the reflux bad. For example, several pieces of pizza with a big tossed salad from the salad bar is a real killer.
I think my Gerd is worse during stressful times. That burning in my throat and bad breath is the WORST. Sometimes it lasts for weeks. Sometimes it goes away. After reading that GERD is a chronic disease, I certainly believe it. I've suffered for six or seven years. Best of luck to you.
