Is is possible to have LPR without pepsin in the throat (rdbiomed pepsin test results)?

Is pepsin in the throat a mandatory requirement to be suffering from LPR? I ordered a test from a UK company, rdbiomed, in which you send 3 saliva samples taken throughout the day. They then measure the amount of pepsin at each sample interval.

The test results came back completely negative. I believe the test is reliable - but has anyone else here taken this test or can report on it? According to the company - the test should be available in the US in 2014, subject to FDA approval.

Ours came back negative too. Type in "rdbiomed" in the forum search engine. Look for comments by JustPassingThrough. He is the one who brought It to my attention and had it himself with "positive" versus our "negative" results. There are many comments throughout the forum regarding the test. I'm sorry but unable to answer your opening question. Kind regards. Pat

Thanks for the feedback. When I asked the company about the validity of the results, they did say I could retry later at a discount.

I still do believe I have LPR. When this started, I a hoarse throat, swelling, lump sensation, etc. I then did the Koufman diet (5 weeks now). It got better at about 3 weeks (along an inconsistent pattern of taking meds) but now I've seen to have regressed a little. My symptoms are now a swollen sensation/lump, voice clearing, etc. My ENT did a flex scope look about 2 weeks ago and said my throat irritation was mild, but sure is a lot more uncomfortable than that.

To be on the safe side, I think the best course is to retake the test (I was offered a discount for a re-test ) in perhaps a few weeks.

The diagnosis occurred when I had an endoscopy in February and was told I had esophagitis, and they immediately recommended taking PPIs / H2.

I can't imagine what else it could be other than some form of LPR.

Hi JDT

I wonder if you would be able to update me on the results of your further Peptest?

I have had the following from RD Biomed:

Pepsin tests: 3 saliva tubes in June 12.
6 saliva tubes Aug 12.
3 tubes March 13 and
EBC test April 13. All on different days, different times, but all negative.

However, scope from ENT confirms moderate inflammation on larynx all consistent with LPR he says. Has no idea what else it could be.

This is soooo frustrating, because I also believe that this is LPR. Tight throat, lump sensation, but no cough or throat clearing.

Did you ever get a positive peptest returned?

Just curious, is this test exclusively available in the UK, or in the US as well?

The response from Mr. Dettmar is very interesting regarding the "negative" results. My test came back negative but I only did the test one time.

Can the epithelial cells ever be repaired? I know that we are trying by drinking alkaline water and black raspberry but are there studies where the cells have returned to a more normal state and healing has taken place?

I am 1 year post linx this month. I am going back to the doctors again, as I am still having problems. I am bound and determined to get this resolved.

Thanks JPT, I appreciate your response. I have posted a question about Evamor water on the main forum, but I see here that there have been other posts about the water. It would be good to know whether it can be purchased in the UK.

I guess I am one of the more 'challenging' patients as all my symptoms point to LPR, but every test is negative except the scope on my throat which shows moderate inflammation. Prof. Dettmar's comments are interesting as the ENT consultant I saw just shrugged his shoulders and had no idea what else could be causing the inflammation. I have had food & allergy testing done, but nothing there. I have never smoked and as for alcohol, well... if only! I have drunk a lot of fruit teas in my time though, I wonder if that could have damaged the lining as he says, but I dont think they were particularly acidic.

I am going to try another ENT I think as he suggests and see what the second opinion yields.

I ordered some more black raspberry today. I have been drinking the Evamor water and still taking GA. Have been off PPI for a month now. Hope I can keep off the meds.

Going to see surgeon at end of this month. Hopefully I can get my point across.

I take psyllium husks twice a day to keep the system flowing too. I also believe it is important to keep things moving. I don't have IBS but I think I might have leaky gut, and I also have many food allergies too.

Could it be bile, rather than pepsin/acid that caused the damage in your throat? Bile is not supposed to be in your stomach, but if some does back wash in there from your small intestine when the pyloric valve opens to let stomach contents out, and you have reflux/regurgitation issues (perhaps silently at night), then the bile could be getting up to your esophagus.

Hi I did the Peptest and two of the three samples came back positive. Prof Dettmar said this proved conclusively that I had reflux although it was probably non/low acid. My symptoms fluctuate so I waited until I had a flare up before I did the test. I've 24hr PH/manomtry my acid levels were low and manometry within normal range although I had 3 abnormal swallows.

I think you need to do the Peptest when you are having full on symptoms, after all the pepsin has to come all the way up to your throat for the pepsin to be present in your saliva.

I've also been told that the Pepsin test is not accepted by Endostim as suitability for this procedure.

I guess a good way to get a postitive PH test would be to eat a large bar of chocolate and a couple of g & t's to bring on a really good flare up, but the thought of the pain is horrible.
