esophyx tif - UK - Hernia repair on its own?

Does anyone know which doctors in the UK perform the esophyx/tif procedure?

Has a hiatus hernia repair ever been done on its own when you have a normal ph study, normal lower oesophageal sphincter pressure, and mild reflux  with small torsional spasms, hiatus hernia less than 1cm - without having to resort to a full fundoplication?

Having the worst pain eating now with this small hernia, and Doctor says it should not be causing any symptoms at all - you just have to live with it.

Having another endoscopy on the 26 July. I could punch the wall with the pain. confused

You're unlikely to get a Hernia repair on the NHS with those results but you may get it privately. Which part of the UK are you?

I understand your pain and frustration, I was also told to live with it by one guy at the age of 13. Maybe you should try someone else.

Thanks Mollymarie - I am in Cheshire and quite happy to go private. I have just spent 6 hours in a&e today with the chest pain and am told by the Dr there that it is not my heart but probably my hernia. It's like pressure builds up in my oesophagus and just causes the darn pain. So the Gastro says one thing and the hospital another - it is so blooming frustrating!

Hello  and welcome to HealingWell.  Glad you've found our online community. It’s a great place to connect with others traveling the same road...you'll find much support, encouragement and advice from others who understand.

I am wondering what tests have been done so far re your issues ?  Most surgical candidates should have the following tests prior to surgery:

 

  • EGD with biopsies
  • Esophageal manometry
  • 24 hour pH study off acid medications (7 days off PPIs; 48 hrs off H2 receptor blockers).

Some Patients may require:

  • Barium esophagram
  • Gastric emptying test

Wow! It's a small world, I'm also in Cheshire!!!!!

I'm sorry to here you were in a&e all day, that must have been quite scary but glad your heart is ok.

It might be worth asking for your ph study and mannometry to be done again. I had one normal ph study and one abnormal one.

If it comes to you seeing a surgeon there are some pretty good guys around here, the one who did mine was great and is very very cautious!

Last year I had 2 endoscopies - the first gastro Dr failed to find the hernia, new Gastro Dr did second endoscopy in Sept found some gastritis and a few polyps that turned out to be fine and a small sliding hernia less than 1cm.

Barium swallow showed small sliding hernia with mild reflux and torsional spasms at the lower end and gastric emptying was slow but steady.

PH 24hr study showed no reflux below 5, with one episode of 8, and was declared normal. Manometry showed oesophagus pressure to be normal but peristalsis did not occur with every swallow.

I seem to suffer with a lot of burping and a feeling of huge pressure in my oesophagus. My breastbone also seems to hurt like ****! However I do also have chostocondritis - which is an inflammation of the ribs at certain areas and that rather confuses the issues. I have had some difficulty at times with swallowing, but i'm unsure if that's the chostocondritis or something else. All I know is that in April last year after a holiday I became ill, got IBS and then a hernia, and I've now got all these horrible symptoms.
I just don't understand why you can't fix the hernia on its own if your sphincter is working properly?

Molly Marie - ooh it is a small world, may I ask what symptoms you have had and what surgery and who with? And how are you doing now?

Symptom wise I had a lot of heartburn and chestpain. Apparently my mum used to find me asleep as a baby surrounded by the contents of my stomach! That seemed to get a bit better and i just suffered from regurgitation, which i always thought was normal. When i was a teenager I started vomiting a lot and developed ulcers in my oesophagus, stomach and duodenum. this was put down to gastroparesis and the ulcers in my oesophagus were from vomiting and reflux. The vomiting stopped after a few years and I was left with the pain.The biggest concern to my GI was when I developed Asthma in the last couple of years. That was when they referred me to a surgeon. Really he should have done it earlier but he didn't do the tests earlier.

The surgeon I saw was Mr Galloway, his private work is at the Alex in Cheadle I believe. Really lovely man, I'd have no problems seeing him again.

I had the full 360 wrap done, best thing I ever did. My asthma and regurgitation have pretty much gone. I'm still have a few swallowing problems but then I had swallowing problems to start with. This causes some pain and a couple of weird sensations but I'm only 6 months post op so pretty early on really and it's not stopping me from doing anything :-)

Hi MollyMarie,

Am thrilled to hear that you are so well now. I am wondering whether just a hernia repair would help me at the moment rather than the full fundoplication at this stage. I am still getting severe chest pain usually after eating and I just don't want to live a life of ppi's and gerd I'd rather be proactive.

I'm going to have to keep emailing surgeons - I might give your Mr Galloway a try. Thanks for all the info x
