Bill, another moderator here, has had 3 Nissens. The first one was done incorrectly, and the second one was damaged as the surgeons were dealing with a perforated esophagus (during and endoscopy), so he is on number 3.
He has had no difficulties with scar tissue causing pain or difficulties at all. He had the surgery done in his 30s. He's busy hunting right now, so he's not available, but hopefully he'll be back soon and will weigh in on your question.
The Nissen continues to be the gold standard for reflux/GERD that doesn't respond sufficiently to medications. It is a surgery that has been around for over 50 years, and has helped many people. It has a great track record and a very high success rate.
Your quality of life and health are very important, and if they can be helped by surgery, then I say, the sooner the better. I waited 6 years because I was fearful of the surgery. In the meantime I was a very sick person, and I have some permanent physical problems because of that long wait.
