If medication isn't working, the nissen might be the next step. I took all kinds of PPIs, and my GI doctor ended up putting me on Dexilant 60 mg; my understanding is this is just about the strongest PPI you can take. It worked to a certain degree, but I was still having symptoms I wasn't happy with. I decided to have the surgery because I didn't want to spend the rest of my life worrying about what was going to trigger my LPR. (Eventually I got to a point where I was refluxing even water.)
Research, research, research. If you decide you want to have the surgery, be sure you find a surgeon who specializes in it. My surgeon had done over 1,000 NF procedures. Also make sure you have all of the necessary tests done. Before my surgery I had an endoscopy, 24 hour pH test, manometry, and barium swallow. They aren't fun tests, but they will tell you if you are a good candidate for the procedure. Sounds like you are on the right track with the endoscopy.
