Deciding whether to get Partial Fundoplication (i.e. Toupet)

So, I've been suffering with reflux for about 4-5 years now (I'm 30 years old). I've typically just managed it with Nexium, and tried to keep a reasonable diet. Lately, though, it has gotten worse. Long story short, my gastro set me up with a consultation with a surgeon a week ago. He was awesome - very knowledgeable.

Because of a manometry I had a few years ago that indicated that I have some swallowing/motility issues, the surgeon recommended a partial wrap (also including a fix to my hiatal hernia). My concern with the partial wrap would be effectiveness; he said that the full wraps have been shown to be more effective in American trials, but not in European trials (He also said that he's not sure if there's a difference). He also mentioned durability/lasting issues with this surgery (i.e. how long it lasts, for how many people it gets undone for, after how many years, etc.).

Anyway, my surgeon wants to some other tests (just wants another Barium swallow done), but basically recommended the partial fundo. surgery for me. On the one hand, I want to have the quality of life that I once had before...GERD restricts what I do in my life, and makes and has made work and school harder. On the other hand, I want to make sure I'm not exchanging my current problems for some other problems (God forbid, even worse problems), to the point where if things go bad that I would wish for my old problems back again.

It's a major surgery. You are basically altering your anatomy. You wouldn't do this unless you are totally sure that you need it. I'm going to be needing to do a lot of thinking. My main concerns are swallowing issues (I want to make sure I can eat normally; Also, the no burping/vomiting thing is really scary (or ability to vomit, but that would mess up your wrap). I don't vomit hardly ever, but it's hard to control yourself when you're sick.

If anyone has any thoughts, it would be very helpful and appreciated.

With a partial wrap, you should still have the ability to burp and vomit (I wouldn't let that factor into your decision). I can tell you with any kind of wrap, you will experience gas and bloating. I had a full wrap 3 years ago, and I can tell you that mine was a complete success, but I will tell everyone that it should be your last resort. Try different medications, change you diet, avoid alcohol, etc. My personal experience was I missed 2 months of work, due to extreme weakness, didn't start to feel great until about 6 months and back to my "new" self until 1 year post-surgery. There are still foods, I avoid, I gave up alcohol and carbonated beverages.
