Sorry to hear about your severe reflux.
PPI strongly inhibit gastric acid secretion, but some amounts of acid are still necessary for digestion. So it is possible (as KarenG mentioned) to have reflux on high dose of PPI.
The main cause of reflux is abnormal function of the esophageal sphincter, a muscle located at the bottom of the esophagus where the esophagus joins the stomach. Surgery wraps the upper part of the stomach around the esophagus in order to prevent reflux. It may be a reasonable alternative when reflux is not controlled by PPI.
Surgery is never the first option, but you may discuss with your doctor what is better for you. Surgery success rate is about 85%.
