The first thing you need to do is make an appointment with your primary care doctor and/or GI specialist. You could have reflux that has irritated your esophagus and caused strictures, that make swallowing more difficult. You could also have a hiatus hernia, as Joy suggested.
Her advice is good...if breads are sticking, stay away from them for a while. Be sure that you eat small bites and chew your food completely. For the time being, only eat foods that can be chewed to a liquid, and chew them to that consistency. If you want carbs, toast the bread (it's easier to tolerate) and enjoy crackers and other crispy carbs. Lots of people here suggested a hot drink with eating tends to help the swallowing go more smoothly. I enjoy tea with my meals. I do notice that ice water can cause some problems for my swallowing.
Good luck finding an answer. In the meantime, try to change your eating habits.
