A wrap creates an unnatural barrier between your esophagus and your stomach. Therefore, you're always going to need to eat more slowly and chew your food more completely, but the answer to your question is no. Things won't always be as tough to swallow as they are at your stage in recovery, but as I said, you'll always have to be more careful.
As Joy said, toasted bread goes down much more easily than untoasted. Actually bread and steak were the two things my surgeon warned me off for at least 8 weeks. Granted, you're at 12 weeks and should be able to handle it, depending on how big your bite is and how much it is chewed, it's certainly possible to get it stuck even at 3 months.
This surgery takes a full 6 months for the majority of healing and a year for the rest (a lot of that is your body adjusting to the changes that have been made). Listen to your body. If it's telling you to take it easy on the untoasted bread, listen.
The rule of thumb is to take small bites and chew, chew, chew, until whatever you're eating is a creamy consistency. That way you'll never have a problem. I'm nearly 4 years post op, and I still have to remind myself to slow down and chew my food more thoroughly. It's a small price to pay for getting rid of the effects of GERD.
