The questions you are asking are what I was asking myself in 2006. I had 3 choices, double an already double dose of PPIs, stop all meds and deal with it, or go for the surgery. My GI recommended the surgery as my symptoms were no longer being controlled with meds and diet and I had developed Barretts.
I also had a very weak LES, so I opted for the Surgery. I chose the nissen because it was what I felt the most reliable at the time. There are a few more choices available now and if you decide on surgery that is a decision only you can make.
If you do opt for surgery be sure to get the tests done to be sure you are a good candidate. A manometry is a very important test to have before surgery.
