Nissen 270

The 270 is called a toupet fundoplication. I'm booked in for a dor fundoplication (180) in 2 weeks. My surgeon and all the others at his practice refuse to do nissens, and haven't done them for a while now. He said that over long term, approx 10 years, toupet and dor fundoplications have a slightly lower success rate, 3-4% more patients have had some recurrence of reflux after 10 years than those who had a nissen. However the chances of having dysphagia, requiring dilations to be done, is about 40% for nissens, also you most likely wont be able to vomit or burp ever again. With the partial wraps, esp the toupet, vomiting will be hard in the first few months after surgery, but eventually you will regain the ability once the swelling has gone down.

As for the length of recovery, he said its about the same regardless of which surgery you get. The way the surgery is performed is equally invasive for all methods, the variation is just in how far around the wrap goes, there is still just as much cutting and stitching done. The swelling is probably more noticeable with the nissen because its the whole way around but the actual time frame for returning to normal diet and activities is the same.

I had a toupet 270 done in March of this year. I was back to work within 3 weeks - and have had no major issues. I can't comment on the vomit issue - as I haven't felt sick since the op (which is heaven - as I used to feel sick a lot!) - but I can belch and have not had any acid reducing meds since my operation. My surgeon prefers to do 270 wraps as he said they were less troublesome than the full 360.

JB - good to see you giving advice - and good to know you are doing OK! I haven't been on here in a while! When things go well time flies by! I agree with you that if I need a re-do in time - I would do it again. I have a social life again
