teen's foot and ankle pain

My 14yr old daughter has had foot and ankel pain for 4 months with only rest, ice, pt, and cortizone shot as treatment.  There has been no relief for pain.  She had an MRI that showed a bruise on the talar dome and a longitudinal split tear peroneal brevis tendon.  Her doctor had her non-weight bearing on left foot for about 6 weeks and re MRIed.  The second MRI showed talar dome was recovering so she was back on left foot and now 8 weeks out she is in more pain.  Dr. said tendon problem was no problem since ankle is stable.  He is now sending us to pain management Dr.  and I asked for a new MRI to see if talar dome bruise is back (daughter says it is) he says just see pain management Dr.  Does anyone have any ideas?  Pain has been bad and Dr. doesn't want to mask symptons.  But he isn't the one living with pain and limited mobility.  Thanks

Hello and welcome to HealingWell.  I am glad you found us and I hope someone is familiar with the type of injury your daughter has.  I do not have an knowledge of this injury.

Was this related to a sport injury ? You may want to get the opinon of another Ortho Dr. I hope the pain management Dr. has some good ideas for you.

I understand why you are worried and I am thinking of you and your daughter.

She has had two ankle sprains in the past but this time she does not remember injuring it.  Thanks hopefully someone will be able to shed some light

My daughter has never torn a tendon (she did sprain her ankle a few times from soccer tho) but I know tendons take a long time to heal. If her doctor is sloughing her off to a pain doc, you can always ask for a second opinion. You need to see an orthopedic doctor if you aren't already doing so and if it's an ortho doc that is referring you to a PM doc; I would definitely get a second opinion.

Hi snybumgarner, I'm new to the site and thought I'd join to so I can reply to you on this thread :)

I'm sorry to hear about your daughter's problem, I'm 17 (18 next wednesday) and have been suffering with a similar problem for just under 11 months now (reassuring I know!) I've had 2 MRI's done and seen about 5 specialists as well as trying a herbal doctor and visiting pain clinics. I have an intense dull pain in my right ankle and it feels like it's coming from the inside of the joint rather than somewhere on the surface that I can pin point. Does any of this sound familiar? This diagnosis was an osteoedema on the base of the talus; which I'm fairly sure is bruising on the bone. The thought is there is a crack in the cartilage which CAN be missed by an MRI.
It's taken 11 months to to rule out all non surgical options and am now waiting for an arthroscopic operation into the joint to investigate any floating debris or damage to the cartilage; unfortunately we can not get the op on private health care so we have to wait for ages for the NHS! This is an old sporting injury which cropped up again after starting intense martial arts and is now severely affecting my life!
I've lost just over a stone due to pain and the medication I take, I'm on tramadol, codeine, paracetamol, naproxen and amytriptiline. I work in a pharmacy and want to be a doctor so I know that's a lot! How ever this is also not controlling the pain and am waiting for another referral back to pain management.
My advice would be to find a orthopaedic surgeon willing to investigate arthroscopically, MRI's will not always give the full picture. Try some milder analgesics as these may decrease pain level and will not mask the pain (not nearly as much as cortisone injections which I've also had and hated!). I hope everything works out okay and she isn't suffering like I am!

my son, 12, woke up about 4 month ago with severe footpain. No injury as far as we know. X-ray showed no cracks or breaks. It took about 1.5 weeks to get an MRI approved. Showed edema in ankle and numerous tendons around ankle/foot inflamed. Not sure if the inflammation was there originally or came as a result of walking funky in orthopedic boot before images were taken. Since then he has had boot, physical therapy which made it worse. Then couple sets of casts, couple more soft-casts. Several sets of steroids, a few weeks ago injection in ankle with no improvement. For the whole time we have been told to give ibuprofen but stopped it a couple month ago because it has done nothing to swelling and concerned about long term effect. New MRI couple weeks ago showed most tendons healed but still osteo edema. Was told if no improvement, they would have to try surgery to see if they can find something that may need fixing. Did not like the idea, so started looking for alternatives. Currently giving acupuncture a shot, also natural anti inflammitory. Was a bit skeptical, but at home my son has been able to put some weight on foot without crutches and with less pain, so will continue that route. Hoping to start physical therapy maybe next week... Not sure if it helps with the edema, but it sure seems to make a difference in pain level. If any of you have suggestions on what else to try, please let me know. It had been very difficult to not move around for that long a time without improvement
