As SC said, hemorrhoids are common as well as fissures, especially with you pushing to have your bowel movement, that GREATLY impacts the occurrence of hemorrhoids and fissures.
The itching, pressure and blood all indicate hemorrhoids, let me help ease your mind here, when you have blood in your stool, especially "bright red blood", and as you said "clots" that means that it is near the exit of your anus area, you have had a colonoscopy, it came back clear, you had a doc look, he said hemorrhoids, and I can tell you without looking at you at all, that you have hemorrhoids and easily fissures.
Here are some tips: STOP pushing when you are going to the bathroom, pick up on more vitamin c, it is a natural laxative and c is good for ya anyway, you can take it by eating oranges or even taking a normal vitamin at least 1000 mg daily, drink more water it helps to keep your stool hydrated and moving along, and if you would like, you can even eat black licorice, and it will definitely make you go, however, it will change the color of your stool to a black/green, do not be alarmed by this at all, its natural. You should have at least one bowel movement per day, it needs to be soft, never mind the shape of it, just as long as it's moving along daily is what you need to be concerned about.
Now let me tell you what causes hemorrhoids, sitting on your duff all day, lifting a lot of heavy weight, gene's, being overweight, lack of exercise and constipation, I probably have forgotten something in there but, I covered the major causes. So, in essence, if you are overweight, lose some, if you aren't getting much exercise, get more and the worst thing you can do is have constipation and or push, never ever ever push it out.
