Have you tried a beta blocker like Propanolol? Beta blockers do something to stop the body from producing large quantities of adrenaline, which in turns prevents heart palpitations and other physical manifestations of anxiety (tension, shaking, sweating, blushing, etc.). I am currently experiencing a LOT of anxiety due to my anxiety med being refilled with the wrong brand (long story, but this particular brand doesn't work for me) and I had been waking up every morning at 5am with heart palpitations. Needless to say it scared me really bad every morning. I started taking Propanolol and it has stopped the palpitations.
If you haven't already, I highly recommend you look into beta blockers. The lowest dose is 10mg and when I take that, I feel a little calmer but that's about it. 20mg seems to do the job for me, but some people need more. I guess it depends on the person.
Unfortunately, beta blockers don't do anything for your mental fears -- but if you aren't having palpitations anymore, perhaps you won't have as much mental anguish. And you can always use your Xanax for that part, anyway! Xanax and Propanolol are like the Yin and Yang of anxiety meds.
