Have you seen a rheumatologist? Bilateral sacroiilitis is the hallmark sign of ankylosing spondylitis. This is an autoimmune disease that needs aggressive treatment to keep you comfortable and to keep the disease from causing more and more damage.
I can't advise regarding the job situation. I will tell you that I am 95% more comfortable and with less pain when I move around. The minute I am forced to sit down and remain somewhat stationary, my pin level increases 10-fold.
