I recall a booklet from ACS that includes recipes designed to pack on calories and protein. I think a lot of them had nuts and peanut butter, which is a yummy way to add both. You may want to contact them, the book had to do with diet and nutrition, but I do not recall the name of it. I do remember reading it though and the recipes looked good.
You may also consider making milk shakes with the Ensure or Boost, adding ice cream and chocolate syrup will help hide the tatste, will giving you a wallop of calories. Of course, cheese is good too. And chocolate cake...
