I may be able to help you out a bit. I had what I assume is the same blood test on my EBV levels when looking for all viruses as a cause of my symptoms. My EBV levels were well over 600. My doctor told me that the lab quit counting after 600 and that it was a sign of a reactivation, as I had mononucleosis five years ago. Any level above 21.9 indicates a reactivated virus. Under 18 means you have not been in contact with EBV in your lifetime. 18-21.9 is where EBV levels should be in patients that have had mono in the past.
He prescribed me Valtrex 1g/2x daily and I have been on it for about a month and a half and have not experienced any relief. I had a blood test Thursday to see if my EBV levels have gone down but have not heard back yet. I also had a Western Blot done and am waiting on that, as I believe it is Lyme due to a low CD-57 count and Lyme symptoms for over a year now.
I feel like I'm rambling and no help but I have read that Lyme can reactive viruses, as EBV never goes away. But so can stress and other factors as the virus is not well understood.
You could ask your doctor what they think about antiviral therapy for the EBV along with the Lyme treatment. I have read that antiviral, antifungal, antibiotic, and supplemental treatments in combination with one another are best in treating Lyme.
