I see that you normally attend the Lyme forum, and I'm very sorry you suffer from this disease, as it can be extreemly hard to treat. I can't offer much in the way of a pain treatment, but did read up on the disease a bit here on the web. Are you on any sort of "Antibiotic"? Also who is scripting your pain med's?, and are you seeing a Dr who specializes in Lyme? Maybe a PM might be a good suggestion for you?
I also added a few general lifestyle suggestions for you below. You might want to check and see if you are following any of them.
You might want to visit with a Pharmacist, and see what they suggest for you. Sorry I could not offer more for you. Lyme is a very complicated disease.

General Lifestyle for CLD (Chronic Lyme Disease)
• Do NOT overdo
• Rest is enforced
• No caffeine - impacts sleep even at small amounts - sleep is CRITICAL
• Balance, balance
• No alcohol - more toxicity
• Stay hydrated - drink lots of water
• No smoking - one cigarette every 3-4 months can impact your recovery
• Vitamins do NOT feed spirochetes
• Exercise is critical
• No steroids
