Have any of your doctor's mentioned complex regional pain syndrome ("CRPS")/reflex sympathetic dystrophy ("RSD")? It is a disease usually (but not always) triggered by a soft tissue injury (can even be as minor as a needle stick) where for some unknown reason, the sympathetic nervous system in the affected area goes haywire. The result is severe pain (disproportionate to the pain of the original injury, if there was one) -- most commonly burning or stabbing pain, temperature changes in the affected area, redness/color change of the affected area, neurogenic inflammation, and muscular/movement abnormalities. It most commonly starts in one or two limbs, and can subsequently spread to other locations. Although, it can occur anywhere in the body (I have it in my eye & face).
There are other members here with CRPS in their limbs, who may be able to provide you with better feedback (my case is somewhat unique/unusual), but many of the symptoms that you mention sound similar to the hallmark symptoms of CRPS. You may want to do some reading about it on your own & see if it sounds like what you are experiencing, and if so bring it up with your doctor. Unfortunately there is no cure (although apparently some patients do go into spontaneous remission), but rather it's a matter of managing the pain and other symptoms.
Hope you are able to get some answers soon. I know how frustrating it can be not to have a diagnosis. It took me over 8 yrs before I finally got diagnosed. Just don't give up. Sometimes it just takes finding the right doctor, especially if it is an unusual diagnosis or presentation.
