It's a bit on the high side for starting doses but I don't know what you were on before. Generally with fent due to its transdermal actions (no turning back) Drs start low to mid point in dosing and move forward. Many people have great success with Fent, no clock watching, smooth and even delivery and coverage, little need for b/t meds. I would however have someone stay with you the first 3day round making them aware that extreme drowsiness and slowed respiration is sign of distress that needs medical attention quickly.
As for drawbacks to fent of course there are a few as with all meds. Important to avoid extreme heat such as spa's, sauna's, hot showers, direct prolonged sun due to "dumping meds" too quickly into your system.
Some have issues getting the patch to stick if they sweat alot, avoid lotions and heavily moisturized soaps and you can get tegaderm at most drugstores that you can use as a added adhesive if you have trouble.
It takes the med some time in the beginning to reach a therapeutic blood serum level so the first day or so may leave you with some feeling of w/d and pain but do not take any other medications.
As I said we have many fent users who have had awesome relief and used fent for many years with a successful outcome.
