I am on Palliative Care (for a severe, life-threatening illness) and also on the Fentanyl patch. I am on 75 micrograms, every 48 hours. I also have break through medication, compounded in liquid form for easier absorption.
With your history of Chron's disease, the Fentanyl patch is a wise choice. You are bypassing the intestinal tract. I had an extensive intestinal perforation due to long-standing use of corticosteroids that thinned the lining of my intestines. As a result, I have an ileostomy. My intestinal tract is not reliable for absorption. The Fentanyl patch is a wise choice for me, as well.
I did find a definitive drop off in the medication into the 3rd day of wearing the patch. The standard dosing of the Fentanyl patch is 72 hours. I now change the patch every 48 hours.
I also found the gel-based patch more effective for me than the mesh-based patch.
The Fentanyl patch gives me better and more sustained pain control than oral-liquid medication. I am grateful to be able to have prescribed access to the patch.
