My PM Dr has lowered my Oxycontin dose because of the new MME regs. Isn't this considered a NEW RX and will not be an EARLY REFILL at the pharmacy? In short, because the Dr changed the dose from 40mg BID to 20mg BID, won't the pharmacy fill it right away?

Hello & welcome to Healing Well. Yes, your pharmacy should honor the new script, if they give you any problems they can call your dr.

Thank you Susie. I'm sure they will honor my new script but will they fill it right away? It's still 11 days to reach that 30 day mark from when they filled my last script of 40mg oxycontin. These pills aren't supposed to be cut in half. So, do you think they will still fill the new one now or make me wait the 11 days?

No they should not hold your fill date at all, this is a new script with different mg's so yes they will fill it now. You need to take it to be filled today, keep in mind you only have so many days to hold a script.

My Dr uses the new electronic method to send all 3 months Rx's to the pharmacy. He always gives me the Oxycontin ER and Hydrocodone for break through pain. But here in Va they've tightened up the law on how much the Dr can perscribe at the same time. So I've been on 40mg, Oxycontin twice a day and 10/325 hydrocodone four times a day for over a year, now my Dr by law has to cut me back. Thanks to all the drug dealers and addicts. I'm not sure what to expect, I just know it's going to be less.

SoberOne, I use to think that the war on narcotics was a result of the druggies, however, my opinion on that has changed greatly. The idiots making these recommendations for the drs these days have an agenda with narcotics period. They could give two hoots about chronic pain patients, we are all lumped into one category. All of their studies about addiction rates & overdoses are convoluted because they use the addicts as a precursor to any study. Look at the publicity a famous gets for overdosing. They usually have illegal drugs on their tox screen with one legal drug & the press has a field day with it. I am all for shutting down pill mills & tossing the abusers out, but not at my expense & that is exactly how its being done. And I will add that some drs are not educated enough about medications & how to properly rx them.

SoberOne, do you know what "new MME regs" in Virginia your doctor was referring to ?

I have not found any new laws in the State Of Virginia stating a doctor cannot prescribe more than a certain dosage of opiates for chronic pain.

The CDC Guidelines are not laws or regulations. Although many doctors tell patients they are. CDC Guidelines are intended for Primary Care Physicians. They recommend Primary Care Physicians not prescribe more than 90 mg Morphine/Morphine Equivalent Daily Dose for chronic pain. The guidelines are not for Pain Management Doctors.

M&G, you are so right about the guidelines but drs are using it as an excuse to the unsuspecting patient. My thoughts only, the drs are going by the guidelines because they do not want to be flagged. And of course there is the insurance companies to deal with & pharmacies.

Susie, one of my biggest pet peeve is for a doctor to tell a patient they can't prescribe opiates, can't prescribe over a certain amount, ect because of new laws when no new law exists. If a doctor doesn't any more respect for a patient that to lie to them. I think the patient should be aware of that.

I understand doctors do this because most patients give them less grief if they think the doctor has no control over it. But, I have seen several patients that wouldn't argue or give their doctor a hard time if you paid them be lied to about this by their doctors.

Chronic pain patients need to stay current on Federal and State proposed, recently signed and new laws. They need to take the time to look them up and read them. Not go by what the news reports at 10pm or others say they say. These laws govern the medications that enable them to function. It should be a priority.

Susie, I mean no disrespect. If I my reply sounds disrespectful, please delete it. We are having heavy rains from Cindy and my pain level has been higher than usual.

Oh M&G, absolutely no offense taken your post by me, lol. I always appreciate your comments & links that you provide to the forum. I completely agree with you that patients must stay informed but what amazes me is so few people actually do. You & I both have read hundreds of posts here from people about this, or suddenly insurance refuses to pay for their meds. It is a patient's responsibility to be an informed patient in every aspect of their care. I just do not know how to get that across to people, they get in their comfort zone & are all happy until there is a glitch. I do think drs use the guidelines as a cop out because they do not want any flack from the patient, however, as a patient we cannot believe every word that comes out of their mouths, I sure don't.

Susie and M&G, these are regs that were adopted by the Virginia Board of Medicine in Feb 2017, following a VirgInia Assembly, which moved quickly through the Virginia Attorney Generals office and approved by him on March 15, 2017 and signed by our POS Governor which immediately put this new law into affect. The new regs mostly effect PCP's but PM Docs have new protocols to follow too. The MME is just a gudline.

I aggree with both of you about the political part of this subject. I'm a retired US Navy man and I've never seen our country so screwed-up. Our new CIC will fix us. I hope and pray. Who knows, maybe I'll trust the VA Medical Docs enough to use them in the future.

SoberOne, I completely agree with you on things being screwed up. We are caught in the cross fire on the political BS. I read an article about a month ago about guidelines & narcotics for veterans. I will only say things are not going to be good for many of the vets, it just blows my mind.

SoberOne, I don't see anything stating there is a limit on what a pain doctor or any other doctor can prescribe. Maybe I'm missing something.

"1. Carefully consider and document in the medical record the reasons to exceed 50 MME/day;

2. Prior to exceeding 120 MME/day, the practitioner shall document in the medical record the reasonable justification for such doses or refer to or consult with a pain management specialist."
