Microlide antibiotics are cytochrome P450 3A4 inhibitors. The antibiotic causes your body to filter the Fentanyl out more slowly, but since you're still putting it in at the same rate, it builds up in your blood and causes you to overdose. Since it normally takes about 17 hours from the time you take the patch off until the medication in your skin is completely processed, you can't/don't stop the "overdose" by just taking the patch off when you become aware of the symptoms.
