I have finally changed doctors and am happy with the new PM doc.  I have been having bad b/t pain.  I am on methadone and oxyir 5mg.  The oxyir 5mg does not seem to be doing the trick when I have a flare up (usually at work where I am on my feet all day).  What should I do?  I used to take oxyir 15mg for breakthrough when I was on oxycontin and it worked well for breakthrough pain.  Should I maybe ask to increase the b/t med or not because I am taking methadone?  THanks

It might take a while with the new PM before he is comfortable with increasing your BT med to anything stronger. Methadone is a great painkiller. Are you taking the BT medication very often? I know that with BT meds, if you are taking them with some regularity that it can be less effective than if you are only taking them when you have severe pain spikes.
I don't know how long you have been seeing this new PM but if it is a recent change, I would hold off a bit on asking him to increase the BT meds.
Are you using ice and heat, to help manage the increased pain levels?

I can't recall - is starting methadone new for you, or has the dose recently been adjusted? Because as Sandi said this being a new PM, he may want to see how you're doing first. If you've just started out on methadone, maybe that may need adjusting before the BT med. is changed.

I think the best thing would just be to call your doctor and report that you're having these flares especially while trying to work, and maybe your PM will suggest adjustments.

I think it's good that you got a new doctor that you like, maybe give these medicines a little longer time to help...
PAlady and Sandi, had some good advise on calling your doctor to report the flares in pain..and go from there...
Sure hope you get a low pain day soon...

I have been on the methadone for a while and am not comfortable with increasing it.  I have read things about methadone that I do not like.  I want to be very carefull with it.  It keeps my pain at bay.  When I have flare ups I use the b/t med.  I only use it when I need it.  That said, I am not going to be in pain and wait until it hurts very bad to use my b/t med.  It is there to be used in instances where I need it so I will.  I will give it a chance and If it still does not do the trick I will let my doc know.  I know we are suppose to titrate our long acting meds so we dont need the short acting ones but I am afraid what happened with my oxycontin will happen with my methadone.  When I took oxycontin my tolerence kept going up and up.  I was at 60mg 3xday and feeling as if it was not working well anymore so I told my doc and we slowly tapered it down and started on the methadone -- 60mg day.  I want to be very carefull.

Shaw, I have been on Methadone for many years...I was on 120 mg. a day along with 30 mg. Oxy IR for BT pain. Recently my Dr. increased the methadone to 150 mg. a day and took away my BT meds. Then I complained and they put me on the 5 mg. Oxy IR...what a joke!! I might as well be eating Pez. My point is that my Dr. never had a problem with giving me the 30 mg. Oxy's so I would talk to your Pm Doc about increasing you to at least the 15 mg. Oxy's... I don't know what mg. of methadone you take, but if it's below the 120 mg. level you shouldn't have any trouble with taking the Oxy's again...
