I'm so sorry that you have to live with this type of pain. It's hard for me to even imagine what it must be like. I do hope the pain doctor give you some relief via medications, if that's what you prefer. You are on a moderate dose of Vicodin and with your kind of pain it's not surprising you are miserable. This is all my opinion, obviously...just thoughts from my own experience.
If you are interested in having another opinion outside the circle of doctors you already have seen, I would suggest you try the closest university/teaching hospital. These folks are on the cutting edge of medicine and technology so you won't get put aside unless there truly is nothing left to do...and that's not an answer they like. At the top of the list of Topics is a thread called Chronic Pain 101. In there is a link to lists of university/teaching hospitals around the world.
