Yes, a difficult situation for you. Fentanyl is an extremely strong pain killer, and a patient needs to be properly opiate tolerant, as a rule, before ever being put on it. Were you on some other opiate prior to the fentanyl?
The fact that you are changing every 48 hours instead the more norm 72 hours would usually indicate a severe pain condition. I was not able to have proper pain control at 72 hours, so my oncologist quickly changed me 48 hours. Been on fentanyl for perhaps 2 years now.
Also odd that the prescribing doctor wants nothing to do with it? Can you explain this attitude, as it would be odd for a doctor to prescribe anything that strong with the potential of lethal side effects that didn't really be looking out in the best interest of the patient.
To come off of fentanyl long term, is also a serious thing, and one needs to be weaned over a fairly long period of time to do it safely with the least amount of strong withdrawal issues.
