So much of this is all on what the surgeon is most comfortable with doing, and exactly what is wrong and how he plans on fixing it! Usually for the cervical spine they often do the anterior approach, but depending on what and how they are doing it, they will also often use the posterior approach too! and some do both during the same surgery. The lumbar area is often done with posterior surgery, if there is a complete discectomy and fusion done, then they will sometimes do both the posterior and anterior surgery. I have heard of going through the side for this surgery, but I don't think it is all that common. Each surgeon has his or her preferred method of doing things, and again allot of it depends on exactly what the surgeon plans on doing! I do know that few spinal surgeons will tackle doing surgery on the thoracic spine, unless it is deemed absolutely necessary!
