hEY Everybody. I'm in my sixth month of the Medtronic Pain Pump. I'd heard stories about give it at least six months to a year and shook them all off - but . . . I started at 7 mcg of dilaudid 75 concentration level - my fist doctor then raised me from 7 to 8.4 or a 20% increase. After 3 months I was at 10 mcg per day. What a joke. I switched to a new physician after 15 years of being with the same physician more out of loyalty or who knows. I am glad I switched. My new doctor took me from 13 mcg a day to 100 the first week. I went back two weeks later and he took my 100 to 435 and then two weeks later went to 675 mcg, The concentration of the dilaudid was 75 when I was getting 7 to 10 a day, then at 75 when I was getting 100 mcg a day then it jumped from 75 concentration to 500 concentration, and then last week when I was titrated up from 435 to 675 the cibcebtratuib hynoed frin 500 to 2000. I haven't been able to find much information about the mme levels of various concentration levels of dilaudid. Is there anyone in the forum who understands these concentration levels. Supposedly a dosage of 675 and power levels of 2000 is enough to kill almost anybody but it isn't affecting me at all. Of course, I've been taking dilaudid orally for over fifteen years my dosage was 32 mg per day with 4 vicodin per day. While I don't feel any pain relief from any of the intrathecal pain pump so far I am optimistic to have found a physician who is willing to help me vs, scared silly from I'd love o hear anybody else's stories. At lea\st you know there are doctors who will titrate up very quickly. DWK

Given your long history of using Dilaudid odds are high you have built up a tolerance to it. Consider discussing Morphine with your dr., it's very much worth trying.

I think I have asked you this before, but will again, what is the dose of Dilaudid being used in your pump, I'm not talking about how much you receive in 24 hours, these are two separate issues.

A vast majority of prior members here that had pumps were for back pain, either with failed back syndrome from surgery or other issues. Whether it was Morphine or Dilaudid in their pumps they had x amount of milligrams in their pumps, & received x amount of milligrams over a 24 hour period, not micrograms.

I am guessing since your condition is related to blood clots this is why your dr is using micrograms with such a low dose. The conversion of 1000 micrograms equals 1 milligram. Knowing this I am not sure where this dr is coming from making such a statement about a lethal dose.

Stray, I am thinking along the same lines I believe you are in using dilaudid in his pump. 15 years is a LONG time to be on one opiate. I’ve been reading these threads since his first one, and I honestly am beginning to wonder if DW understands the complications/implications of his long term use of dilaudid orally and now in the pump and why he likely isn’t getting benefit of pain relief from the pump, despite large bumps in how much he is recieving. It is and will however continue to increase his tolerance to dilaudid.
I honestly don’t think no matter what dose is used he’s going to find benefit from this pump if it’s filled with dilaudid.
Going higher and higher in concentration/mcg ISNT going to solve the issue or bring pain relief.
I think, like you, the better option would be a complete change to another opiate and start over with slowly tut rating upward.
