When you say it looks like a piece of swelling hanging out, that sounds to me like a prolapsed internal hemorrhoid (you might be able to find a picture on the Web). There's a good chance you can get this to calm down on its own. One thing that can help is to try a low-residue diet temporarily, in order to have fewer bowel movements (they can push the hemorrhoid outward and you want it to reduce in size, and recede back inwards). Also, you can try ointments like you used before to help reduce the inflammation. One thing, doctors will sometimes recommend surgery for treating hemorrhoids. Avoid surgery for hemorrhoids if at all possible if you have Crohn's disease, because the results are usually not good.
